WNY BloodCare Innovation Sprint, September 8th through October 6th, a virtual experience
In a 4-week, virtual Innovation Sprint sponsored by Blackstone LaunchPad at the University at Buffalo in partnership with WNY BloodCare and UB’s Office of Interproffesional Education, students will develop innovative solutions to a WNY BloodCare challenge:
Collaborating in teams, students in a 3-5 member group will develop a proposal to address:
How might WNY BloodCare create a robust “transition to independence” management plan for high school students (Grades 9 – 12) to facilitate these patients’ transition to independent living at college? How might WNY BloodCare assess the effectiveness the transition process?
Students will be asked to specifically consider:
- Preparing patients and parents for the transition to independent disease management;
- Introducing patients to working with their healthcare team as an independent adult;
- Ensuring patients have robust self-management skills;
- Transition plan must be a short, efficient tool that can be used to prompt discussion with the patient at annual clinic visits;
- Preparing students for college readiness; and
- Reporting mechanism for development and implementation of the transition plan.
